Add a Lookup Table
You can create a lookup table for reference from a service type(s) or reimbursement term(s).

- Access the Add Lookup page.
- Select Go To > Contract Loading > Lookup Table Management. The Lookup Table Management page opens.
- Click the Add Lookup Table link. The Add Lookup Table page opens.
- Select the Table Type.
- The type of table you select determines the fields that display on this page.
- For more information about table types, refer to Lookup Table Types.
- Type the Table Name.
- You cannot create a table with the same name and effective date of any lookup table in your facility.
- If you create a procedure lookup table with the same name as an existing lookup table but different effective date, the new table is assigned the same code type as the original table.
- Click Check Name to compare the name you typed to the lookup tables in your facility or facility group.
- Optionally, select an Effective date range.
Note: This field is not available if you selected CDM as your table type.
- Select the Code Type.
- This field is only available if you selected Procedure Lookup or Provider Specialty Percentage Lookup as your table type.
- For Provider Specialty Percentage Lookup tables, the code type allows you to select Provider Type or Provider Specialty.
- For Procedure Lookup tables, the code type allows you to select the type of procedure code.
- Select the Procedure Type.
Note: Procedure type options are only available if you selected Procedure Lookup as your table type.
- Click Add.
