Request Forms
The following forms are conveniently located online:
- Use the Contract Manager Contract Action Request Form to submit information needed for FinThrive to create or change a contract profile.
Note: This process has been relocated to the Community Portal. Click the link on the form to create a case to add or edit a contract.
- Use the Contract Manager Client Loaded Contract Action Request Form to submit information to FinThrive for a contract profile that you have created or changed.

- Use the Contract Manager Internet Access Request form to submit information needed for FinThrive to create or change a user’s access.
You can access a form online, complete it, and return it to FinThrive. This option saves the time it would take to contact FinThrive, request the form and wait for it to arrive before you get started.
Procedure
- Select the form you want to print from the Help > Request Forms menu.
- Select the form you want to use:
- Click Contract Action Request to open the Contract Manager Contract Action Request Form.
- Click Client Loaded Contract Action Request to open the Contract Manager Client Loaded Contract Action Request Form.
- Click Internet Access Request to open the Contract Manager Internet Access Request form.
The forms are in a PDF format. You can use Adobe® Reader® to complete the form, or you can print the form and complete it manually.
- Complete and sign the form, then send it to FinThrive.
- Authorized users can digitally sign the Contract Manager Contract Action Request Form and the Contract Manager Client Loaded Contract Action Request Form, then email it to FinThrive.
- Authorized users can also sign these forms manually, then email, fax, or mail them to FinThrive.
- Authorized users must manually sign the Contract Manager Internet Access Request form, then email, fax, or mail it to FinThrive.
