Reimbursement Type

Reimbursement type, either expected reimbursement or RA, if applicable (the latest RA is shown if there are multiple RAs).

Service Date

Date the service was rendered.

Revenue Code

Revenue code.

Billed HCPC/CPT Code

Associated CPT or HCPC codes billed to the account

APC Code

Ambulatory Payment Classification codes.

Fact Code

Current coverage/price factor for the procedure.

Payment Status Indicator

APC payment status code (see the Payment Status Code).

Billed Units

Number of units billed.

Covered Charges

Amount of charges covered by Medicaid.

MN Fee Schedule Payment

Fee schedule rate.

Total APC Payment

Payment by APC for the line item.

Expected Payment

The calculated reimbursement based on the predefined contract terms and conditions and service type definitions in the system and the repriced claims data.

Reason for rejection or return, if applicable
