Billing Type Definition

Bill types (field locator 4 on a UB-92 or UB-04) have specific meanings on the Universal Billing form. The three-digit bill type is divided into three sets of information.

  • The first digit identifies the type of facility
  • The second digit identifies the bill classification / type of care being billed
  • The third digit identifies the sequence of the bill within the episode of care
